Health Action Planning
Valuing People says that personal health action plans would help people with learning disabilities to get their health needs met. It said that everyone with a learning disability would have a health action plan by June 2005.

Health Action Planning and Health Facilitation Events
The Department of Health and the Association of Practitioners in Learning Disability (APLD) held conferences to help people think about the key messages and actions around Health Action Planning and Health Facilitation. The days were a mixture of presentations and group work. The events were for members of Partnership Boards, including people with a learning disability, carers and health and social care workers.
