Welcome to the new Health Self Assessment page. This page may grow in the course of the next year or two if people want us to include new links to additional information. In the meantime, though, our aim is to make it easy for you to get the papers you need to do your annual self assessment in the coming year.
From 2010, all localities in all ten regions in England will be doing this annual learning disabilities health self assessment.
If you have any questions about what is happening in your region about the self assessment - and when - you can contact your Regional Valuing People Now Programme Lead and/or the manager responsible for this at your Strategic Health Authority. Their names and details are shown at the bottom of this page.
This will give you more information about the papers below and how to do the self assessment. For people who have already done Year 1, it highlights the changes this year to the Framework and linked papers.
People who had been working with the Framework in different parts of the country met to review the Framework and linked papers in November 09. You will need to read the revised 2010 Framework carefully to be aware of the changes. The Guidance Notes will also tell you more about the changes made.
Appendices to the Framework
There is an additional (sixth) appendix this year about the Annual Health Check. The Guidance Note will also tell you more about the appendices
This form allows you to record the level of progress you feel you are at after your self assessment exercise. It will also allow you to record the really goof things you are doing, as well as your priorities for the year ahead. Once completed, your PCT returns it to the SHA.
This additional form has been included this year at people's request, in order to pull out more clearly the data/measures requested - and also to be more specific about the suggested evidence or supporting information people should provide. Again, it should be completed as indicated, and your PCT returns it to the SHA.
